If you prefer, we’re happy to have a member of the Alliant Home Health and Hospice Care team come and meet with you in person to collect your information and discuss your needs.

To help establish a high quality, comprehensive plan of care, we will request the following information upon referral:
- Patient demographics
- Insurance name and policy number
- Physician order for home health
- Patient medication list
- Pertinent clinical documents (such as a history and physical)
